Adults = 21-64 yrs. Teens & Young Adults = 13-20 yrs. Teens and young adults can be added to an adult plan for $20/month

For Virtual-only Care

• Adults | $50/month

There is a one-time enrollment fee of $100 for adults/$150 for couples or families.

Care is led by a licensed, Board-Certified Nurse Practitioner and a care team focused on whole-person, evidence-based primary care.

We serve patients 13 -64 years old.

Yes. Memberships are month-to-month unless otherwise stated.
